ARRIVAL TIME

We ask that you kindly allow time before your appointment to complete paperwork or update your information.

If you are an ESTABLISHED PATIENT, please arrive 15 minutes prior to your scheduled appointment time.

​If you are a NEW PATIENT, please arrive 30 minutes prior to your scheduled appointment time.

You can also come by our office to pick up this paperwork in advance. This will improve your check-in time and allow us to see you and other patients in a timely manner.

PLEASE NOTE

Arriving 10 minutes late for a scheduled appointment is subject to cancellation. We want to be compassionate and understanding to all of our patients and ask for your cooperation to help us with our efficiency.

Please notify our office 24 hours prior if you need to cancel your appointment for any reason. Please also review the No Show/Cancellation Policy page.

Please understand that this is a subspecialty ophthalmology practice. At times, please be prepared to spend two hours or more per visit.

  • Up-to-date Insurance Card
  • Government Issued Photo Identification
  • List of all medications
  • List of pertinent medical and surgical history
  • All eye drops that you are currently taking
